Phd Creative Writing Programs

Which universities in the U.S. offer a PhD in Creative Writing?
I am interested in a PhD in Creative Writing, but I don’t know which schools offer the program. The Georgia State University website says that only seven schools offer both the MFA and PhD in Creative Writing. Which schools are they? (I know three- Georgia State, the University of Wisconsin-Madison, and the University of Nebraska- Lincoln.) What have I missed? Also, any schools that just offer a Creative Writing PhD?

Ohio University
Plus, many other schools offer Doctorate programs in English or Fine Arts with a focus/dissertation on Creative Writing.
